To save time during your initial visit, we have provided forms to download and fill out. Completing these forms before your appointment will allow us to treat your condition more efficiently. Complete the forms online or at home and bring them with you to your appointment.
Pre-Appointment Intake Form
Referral
Your insurance company may require a referral from your PCP to see one of our specialists. It is your responsibility to obtain this prior to your appointment. Your appointment will have to be rescheduled if we do not receive the referral.
Insurance
It is best to become familiar with your insurance plan and coverage. Please refer to your provider directory and benefits guide. Some health care services may not be covered under your plan. Our office requires payment at the time of service. This may include your deductible, co-insurance and/or co-pay. For your convenience, we accept cash, personal checks, MasterCard, Visa, American Express and Discover.
Disability / FMLA
Please be informed that we require 5 working days to complete disability and physical examination forms. Depending on the complexity and time required, a document completion charge may be applied.
Refills
Please call your pharmacy for prescription refills. Refills called in to our office by pharmacies are processed between 8:15 a.m. and 4:30 p.m. weekdays. Refill requests are not taken after business hours, weekends or holidays. For your protection, refills are not processed for patients that have not been seen in our office within six months.
Narcotic Prescriptions
Narcotic prescriptions are usually prescribed for a few days or a few weeks at a time for a specific condition. These medications should be used as directed. Lost narcotic prescriptions will not be replaced.
